At the Greater Rochester Health Foundation, we’re committed to supporting these communities in building on their strengths by providing resources and tailored support to meet their goals for health and well-being.
 

We’re committed to showing up, listening, and strengthening our relationships. Every rural community has its own unique approaches and strengths. In efforts to deepen how we support rural communities beyond grantmaking, we’re building this Rural Health Resource page, and we need your help!

The following resources were highlighted during the 2025 National Rural Health Association (NRHA) and Grantmakers in Health (GIH) Rural Health Convening, where Greater Rochester Health Foundation joined other foundations and tribal leaders in exploring ways to build equitable partnerships in rural and Indigenous communities.

    A hub for federally funded research, toolkits, and publications on rural health outcomes and systems.
  • Rural Health Information Hub (RHIhub)
    Funding databases, toolkits, and practical guides for rural nonprofits and health systems, including an “Am I Rural?” location tool.
    • RHIHub: Social Determinants of Health for Rural People: Updated topic guide provides an overview of the social determinants of health for rural populations, offering resources on factors like healthcare access, education, housing, and employment, along with programs and models to improve the conditions where rural people live, learn, and work.

    Comprehensive resources on rural community development, including economic, infrastructure, and health initiatives — a valuable source for understanding rural policy and funding landscapes.

Additional Resources

  • HRSA (Health Resources and Services Administration) grants webpage provides comprehensive resources for organizations seeking funding to support health initiatives that benefit rural communities and underserved populations across the United States.

  • Visit the Rural Partners Network to browse and find resources developed exclusively for a range of rural communities, individual residents, Tribes, Native Americans, and Alaska Natives. In addition, find programs listed in the program listings on the “Help for Rural Communities” section of the website.

  • The2024 Rural Child Care Joint Resource Guide was developed in partnership with the USDA Rural Development and the U.S. Dept of H.H.S to provide useful information to address the need for improved access to affordable, high-quality childcare and early learning facilities.

  • The Veterans Administration (VA) provides VA Video Connect | VA Mobile for telehealth appointments. This brochure highlights a Telehealth access site in Orleans County, allowing veterans to attend virtual appointments with their providers on a computer with a camera, eliminating the need to travel up to 30 miles to the nearest VA facility.

  • Cultures Learning Together in Geneseo, is sharing health resources in multiple languages, including COVID information and details about the State’s health plan marketplace.
