Heart and Soul Community Church

Little Bit of Coffee & a Whole Lot of Jesus – Expand church initiative to provide a drop-in site for people in the N Clinton area who are addicted, homeless or have mental health problems. The drop-in will expand from one to two days per week, include a hot breakfast and have a social worker on site. County served: Monroe. Awarded: $7,055.

 

Aenon Missionary Baptist Church

Aenon Healthy Minds and Bodies Ministry 2 – Expand and enhance fitness programming for congregation members with a focus on those over 50 experiencing diabetes, hypertension and other chronic diseases. County served: Monroe. Awarded: $15,115.

 

Corfu United Presbyterian Church

Pembroke Central Schools Bagged Meal Program – Expand program providing weekend food for low-income children in the Pembroke district to include students in the primary grades. County served: Genesee. Awarded: $5,000.

 

Rochester Black Nurses Association

Rochester Black Nurses Association: Bridging the youth mental health gap – Youth Mental First Aid (YMFA) program training for members of Rochester Black Nurses Association (RBNA), selecting 6 members to further train as certified YMFA instructors. These instructors will then be available to train staff/volunteers in organizations who are working with youth at risk of mental illness. County served: Monroe. Awarded: $12,540.

 

Bizy Bodyz Inc

Bizy Bodyz Youth Dancers – Structured dancing lessons for primarily African American low-income children, aged 4-19 from western part of the city. Dances types include: hip hop, ballroom, jazz, modern dance and ballet. Classes are three times a week at two community locations and are also offered during school recesses and summer. County served: Monroe. Awarded: $11,210.

 

Tilc Ministries Inc.

Silver Foxes (SF) Program – Provide a program of light physical activity, cooking, gardening and socialization for up to 20 elderly EMMA residents, aged 72-94. Programs meet three times a week at the TILC center where seniors already gather for Foodlink distributions. County served: Monroe. Awarded: $10,150.

 

Catholic Charities of Livingston County

BackPack Program – Expand to five Livingston County School districts the backpack program providing nutritious food to low income students on weekends and school holidays. Funding is also requested for a PT program manager to pursue sustainability of the program. County served: Livingston. Awarded: $19,896.

 

Horizons at Warner

PATH: Promoting Adolescent and Teen Health – Provide additional health and fitness activities for low income middle schoolers attending the 6-week Horizons summer enrichment program on the UR campus. Campers will choose from fencing, Taekwondo, African dance, Zumba and strength training. County served: Monroe. Awarded: $5,400.

 

YWCA of Rochester and Monroe County

YWCA Health Improvement Initiative – Increase the health and safety for women and families by replacing worn out, unhygienic carpets in their seven emergency family units. County served: Monroe. Awarded: $14,000.

 

AutismUp

iCanBike – A week-long skills development program to teach children, teens and adults with Autism to ride bicycles. People with autism have difficulty with motor skills and coordination. They will be collaborating with volunteers and iCan Shine, who specializes in instructing those with autism cycling. Counties served: Genesee, Livingston, Monroe, Ontario, Orleans, Seneca, Wayne, Wyoming, Yates. Awarded: $5,547.

 

Redeem Bethel COGIC

Healthy Living Is Ageless….It Never Grows Old – Support a variety of health and wellness activities targeted to the senior members of Redeem Bethel’s congregation. Program elements include: physical exercise, education on nutrition/portion control, weight management, medication management, continuation of high blood pressure reduction effort. Dance Studio Flooring Upgrade for Healthier Dancers 2 – Upgrade and replace flooring in community dance studio to industry standard, sprung surface to offer dancers a safer dance studio that more proactively protects against injury. Jewish Family Service of Rochester, Inc.

Increasing efficacy of operations at The Brighton Food Cupboard: Equipment and technology upgrade – Upgrade technology and equipment to improve and streamline Brighton Food cupboard processes for ordering, delivering food orders and tracking program clients. Grant would be used to purchase tablets, a bar code printer and develop an app for volunteers to use when processing client orders. County served: Monroe. Awarded: $12,200.

Summer Paddling at GWC – Expand partnership with summer camps to increase the number of youth paddling in the summer, sponsor paddling excursions on the Genesee River and Erie Canal, and a free paddling expo in the heart of the city where city families, youth, and adults will be able to see what the river has to offer them. Focus on the Children

Beds and Cribs – Provide new beds, cribs, mattresses, and bedding for families who are financially unable to provide a safe, separate sleeping space for their children. County served: Livingston. Awarded: $10,000.

 

Zion Fellowship

Supply A Smile – Provide replacements for missing or extracted teeth and dental services not covered by donations for uninsured or under-insured adults in Ontario County at no cost to the patients. Free dental care provided by dental professionals include exams, x-rays, cleanings, fillings and extractions, but funds for extractions are lacking. Awarded: $7,000.

 

South Presbyterian Church

Monroe Milers – Urban-suburban youth running program requests funding for youth running gear and supplies, race entry fees, volunteer mileage and recognition. This 8-year old program is run by volunteers who run with and coach youth ages 6-18 for practices and races over two, eight-week sessions during the year. Awarded: $8,665.

 

Refugees Helping Refugees

Health Literacy program for refugees with 4 components 1) a Speakers’ Bureau; 2) a combined health literacy and exercise class (“Healthy Living”); 3) A hot and healthy lunch, prepared by coordinator and a group of students, and 4) Access to fresh produced a mobile food truck. Breast Cancer Coalition of Rochester Inc. Pilot a trial of mid-week, daytime, and Saturday healing arts classes (Tai Chi, Yoga, and Qi Gong) for breast cancer patients to reduce stress and promote wellness. Amount Awarded: $8,735. Counties Served: Monroe.

Jewish Family Service of Rochester, Inc. Pilot a safe sleep education program for parents of newborn babies at Unity Hospital. Pilot includes education provided by nursing staff prior to discharge, a take-home safe sleep kit and follow up reinforcement from the child’s pediatrician. Amount Awarded: $12,000. Epilepsy-Pralid, Inc. Assist families to start and maintain a ketogenic diet for their children in an effort to reduce seizures. The special diet has shown effectiveness in children with medication-resistant epilepsy. Amount Awarded: $7,271. Counties Served: Monroe.

Greater Rochester Community of Churches. dba Faith in Action Network Assist people with no resources and no family to document their choices/wishes for end of life care and burial, and to access hospice care. Amount Awarded: $5,000. Counties Served: Monroe.

CURE Childhood Cancer Association Add a second parent advocate to provide emotional support, educational, and financial assistance to families with children undergoing treatment for cancer or sickle cell anemia. Generation Two: Creating Intergenerational Friendships, Inc. Program matches trained adult volunteers with K and 1st-grade students at RCSD School 3 for weekly, thirty-minute one-to-one child centered visits to promote social and emotional strength and resiliency for children who have had many adverse experiences. RESOLVE of Greater Rochester, Inc. Pilot Intimate Partner Violence (IPV) Virtual Health Coach, an interactive software application that will facilitate patient education about the health and safety effects of IPV and assist patient connection to community resources. Earthworks Institute, Inc. Pilot a therapeutic wilderness program to address the mental health issues of veterans enrolled in Warrior Salute, a local residential rehabilitation program. Cayuga/Seneca Community Action Agency, Inc. Pilot to assess feasibility of a volunteer transport service to address gaps in transportation service for rural county residents with medical and health related appointments. Amount Awarded: $3,184. Counties Served: Seneca.

Jefferson Family Health Fund Provide physical exercise activities for patients and neighbors of the Jefferson Family Medicine practice in the South West area of the City of Rochester. Kids Thrive 585, Inc. Pilot project to assist families in locating and accessing community resources needed to support their family and child’s basic needs such as housing, food, and domestic violence services. Amount Awarded: $15,000. Counties Served: Monroe.

Partners in Deaf Health, Inc. Pilot to translate Emergency Department discharge instructions to an American Sign Language video format to improve communication with deaf patients and increase their understanding and follow through with post discharge care. Friends of the Rochester Public Market Provide chartered transportation for low income residents in Rochester Housing Authority housing sites to shop at the City of Rochester Public Market and use their SNAP benefits to purchase fresh fruits and vegetables. St. Mark’s & St. John’s Episcopal Church Build and support raised bed gardens at the home of five neighborhood families who do not have enough resources to purchase fresh vegetables. Amount Awarded: $2,426. Counties Served: Monroe.

Cayuga/Seneca Community Action Agency, Inc. Expand the South County Transportation Corps (SCTC) to provide rides for residents throughout Seneca County who need access to medical and dental appointments. Amount Awarded: $4,406. Counties Served: Seneca.

South Wedge Planning Committee Pilot program to provide education on healthy food choices and practical ways to use accessible fresh food for tenants in two low income apartment buildings in south east Rochester. Pilot will include on site cooking classes and coupons to increase ability of residents to purchase fresh foods. Amount Awarded: $2,830. Counties Served: Monroe.

ROC SALT Mission Center / South Wedge Food Program (a project of the Presbytery of Genesee Valley, Inc.) Food Cupboard would Purchase and stock incontinence products for low income people who suffer the physical, social and health effects of incontinence but do not have the money to purchase these items. Project will include counseling and follow up with clients re use of items and developing strategy/contacts to generate product donations. Amount Awarded: $5,000. Counties Served: Monroe.

PROGRAM IMPROVEMENT

Genesee Valley Health Partnership, Inc. The University of Buffalo’s Institute on Trauma and Trauma-Informed Care (ITTIC) will train and provide technical assistance to 25-30 staff members from Livingston County schools and community-based organizations to improve their skills in serving youth and families. Amount Awarded: $10,523. Counties Served: Livingston.

S2AY Rural Health Network Rural Health Network will acquire GIS software and training to map and analyze health and disease data and trends to support public health planning and delivery in five rural counties. Amount Awarded: $20,360. Counties Served: Livingston;Ontario;Seneca;Wayne;Yates.

Mental Health Association of Rochester/Monroe County, Inc. Implement additional twelve-week sessions of the Prime Time Sisters Circle, an intervention designed to motivate African American Women to reduce obesity and chronic illnesses by a combination of health education, behavior change and social supports. Albion Running Club Provide a 12 week training program for novice runners to culminate in completing a 5 k or greater race and discounted running shoes to eliminate potential barrier to running. Amount Awarded: $17,000. Counties Served: Orleans.

Muscular Dystrophy Association Expand support group and informational programs for adults and teens with neuromuscular diseases, their caregivers and families. Amount Awarded: $7,065. Counties Served: Monroe.

Bivona Child Advocacy Center Monthly support group sessions and as-needed emergency debriefings for staff of twenty-one partner agencies who investigate and prosecute child sexual abuse and serious physical abuse cases and support child victims. Amount Awarded: $6,891. Counties Served: Monroe.

Veterans Outreach Center, Inc. Provide intensive training in trauma-informed care for the entire VOC staff and key volunteers, and purchase diagnostic measurement tools for clinical staff to improve screening and response to veterans with symptoms of post traumatic stress and related disorders. Center for Teen Empowerment Integrate health and wellness focus into the Teen Empowerment program with a variety of new activities and changes to existing program to include physical and mental health wellness practices and opportunities. Amount Awarded: $15,059. Counties Served: Monroe.

13thirty Cancer Connect, Inc. Support wellness activities (yoga, physical fitness, art therapy, massage) for adolescent and young adult cancer survivors to increase coping skills and provide peer support. Amount Awarded: $4,935. Counties Served: Monroe.

Penfield High Schools’ Life Skills and Post Graduate Program Increase weekly physical activity and improve nutrition of disabled students ages 12-21 years enrolled in Penfield School District’s life skills and post graduate programs. Amount Awarded: $6,000. Counties Served: Monroe.

Mental Health Association in Genesee/Orleans Counties Train community members in consumer oriented and public jobs (schools, law enforcement, librarians, business) as Mental Health First Aiders, equipped with the knowledge and skill to provide initial help to people in mental health crisis before appropriate treatment is provided. Black Physicians Network of Greater Rochester Inc. Promote emotional and physical well being of black medical residents and fellows who experience high levels of depression, symptoms of burnout, and low quality of life. St. Joseph’s Neighborhood Center, Inc. Two year project to address structural racism within organizations providing health and behavioral health services to low income poor people of color and graduate students preparing for work in poor communities. Refugees Helping Refugees Peer-driven refugee program to address the problems of health literacy, sedentary life style, inadequate nutrition, and limited English proficiency among resettled refugees. Amount Awarded: $25,570. Counties Served: Monroe.

Judicial Process Commission Train staff and mentors working with ex-offenders in Cognitive Behavioral Interventions to better assist these men and women to address mental health and substance abuse problems, and to prevent relapse and recidivism. Amount Awarded: $24,871. Counties Served: Monroe.

National Audubon Society, Inc. DBA Montezuma Audubon Center Provide active outdoor recreation and environmental education programs for Clyde-Savannah and North Rose-Wolcott middle and high school students for six weeks of school vacation during the 2017/2018 school year.
